Care Manager

A Renewed Mind is a private, not for profit 501(c)(3) behavioral health care organization. We are currently looking for qualified individuals to fill a Care Manager position.  This position is responsible for providing psychiatric supportive treatment services in the community at locations convenient to the persons served.

Essential Duties and Responsibilities Include (but are not limited to):

  • Monitor the needs of persons served's mental illness or substance use, physical symptoms, as well as their response to treatment.

  • Work with families of persons served to identify needs and connect with resources to meet those needs.

  • Coordinate community resources including school, probation officers, referral sources, and outpatient mental health providers to ensure continuity of care.

  • Facilitate appointments to ensure overall physical well-being, including basic, mental health, and substance use needs.

  • Achieve productivity goals as assigned.

Education/Experience/Licensure/Certification:

Required:

  • High school diploma and

  • Three years of relevant experience

  • CDCA

Preferred:

  • Bachelor's degree in a related field
